Enrollment Verifications
Enrollment verification is the release of academic information from your records to a third party or parents. To authorize this release of information, the student must fill out one of the Authorization for Release of Information forms.
Why?
This form is required for Santa Barbara City College Admissions & Records to release and confirm information for the following reasons:
- insurance coverage
- medical coverage
- international student travel
- housing authority
- etc.
How?
Steps to submit form:
- print out required form from the link provided
- complete form including signature
- bring form to Admissions & Records Window 5 along with a photo ID
