Student Request for Accommodation

To request an accommodation for disability please submit documentation of your disability to the DSPS office in the Student Services building.

You must also provide answers to the following questions:

  1. What is your name?
  2. What is today's date?
  3. What is your phone number?
  4. What is your email address?
  5. What accommodation(s) are you requesting?
  6. What is the course name, number and section number?
  7. For what semester and year?
  8. What is the name of the instructor of the course?
  9. Is the course online or in a classroom?
    • For courses in a classroom, what is the room number, day and time you meet?
  10. When did you submit your disability documentation to the DSPS office?

A DSPS counselor will contact you and your instructor to discuss your request.  This interaction must take place in order to reach agreement on your accommodation.
